    Introduction

    Primary Carers who have been categorised in Priority Group 3 must submit a QEIIMC Primary Carer Statutory Declaration to the Parking Department to substantiate their primary carer status. A Statutory Declaration is to be submitted as an addition to an existing staff parking permit or when submitting a new staff parking permit application. Primary Carers of children are required to submit a completed Statutory Declaration form to the QEIIMC Parking Department and include the details of the number of children and their respective birth dates. Without this document, the claimant will remain in Priority Group 3.

    Primary Carers of a person 16 and over, with a disability or a long-term health condition are also required to submit a letter from the patient’s doctor detailing the health conditions, living circumstances and the level of care required. Without this supporting document, the claimant will remain in Priority Group 3.

    Staff seeking parking access to the QEII Medical Centre based on a medical condition or disability must submit a QEIIMC Medical Statutory Declaration. This is to be submitted as a variation to an existing staff parking permit or when submitting a new staff parking permit application.

    Definition of a Primary Carer at QEIIMC

    For Children Under 16 Years of Age:

    A staff member is recognised as the Primary Carer if they have the greatest responsibility for the child/children's:

    • Day-to-Day Care: This includes activities such as feeding, dressing, bathing, and overseeing medical appointments and other similar activities.
    • Welfare: This includes housing, well-being, transport, medical attention and other similar activities.
    • Development: Facilitating the intellectual, psychological, and emotional growth of the child up to adolescence.

    For Adult Dependents with a Disability or Long-Term Health Condition:

    A staff member qualifies as a Primary Carer for an adult dependent with a disability or a long-term health condition who is not living in a respite or other care facility, who provides (and is likely to continue to provide for a further six months) the majority of assistance to the person with day-to-day care and one or more of the following core activities:

    • Day-to-Day Care: This involves supporting the dependent in their daily activities and needs.
    • Core Activities: Including assistance with mobility and communication, ensuring the dependent's basic needs are met.

    Guidance Notes for Determining Primary Carer Status:

    • Singular Primary Carer: At any given time, only one person can be recognised as the Primary Carer for an individual.
    • Equal Care Situations: In cases where multiple individuals (e.g., parents) provide equal care, any one of them can nominate as the Primary Carer. However, only one will be officially recognised as such.
    • Specific Care Aspects: The role of a Primary Carer encompasses various aspects of care and support, ensuring the well-being and development of the dependent.

    Primary Carer Statutory Declaration Requirement

    To apply for primary carer parking considerations at QEIIMC, staff are required to complete a Statutory Declaration. This declaration is a formal acknowledgment of their role as a primary carer. It serves to substantiate their need for parking arrangements, ensuring that this support reaches those who genuinely need it. The declaration form is accessible online, and completed forms should be submitted via email qeii.parking@health.wa.gov.au to the QEIIMC Parking Department.

    Completing the Primary Carer Statutory Declaration:

    The following guidelines should be adhered to when completing the declaration:

    For Children Under 16:

    1. Primary Care Responsibilities: Clearly detail how you provide primary care to your child or children. Examples should include day-to-day activities like feeding, dressing, bathing, and handling medical appointments. Please note, whilst transporting your child/children is an aspect of Primary Care, it does not classify you as a Primary Carer.
    2. Percentage of Care Provided: If you are part of a couple, explain how/why you are responsible for the majority (over 51%) of the primary care. For single parents, this should be stated on the form.
    3. Child's Details: Include the full name(s) and date(s) of birth of your child or children.

    For Adult Dependents:

    1. Dependent’s Circumstances: Provide information about their living situation e.g. If they live by themselves/in the care of others, their capacity for self-care, or if they are in the care of others.
    2. Medical Verification: A letter from a doctor or Specialist is required, stating you are the primary carer, outlining the medical condition(s), care level required, why they are unable to care for themselves, and the expected duration of this need.

    Medical Considerations

    To be eligible for staff parking access based on a medical condition or disability, you must be able to demonstrate that your mobility to and from the QEIIMC is severely restricted by a permanent or temporary medical condition or disability.

    Medical Statutory Declaration Requirement

    • Required Documentation: To access the medical condition or disability parking considerations at QEIIMC, staff are required to complete a Statutory Declaration. It serves to substantiate the need for special parking arrangements, ensuring that this support reaches those who genuinely need it. The declaration is to be submitted in conjunction with a new or renewal staff parking permit application.
    • Confidential Review: Each application is assessed with discretion and confidentiality, focusing on the specific medical needs outlined in the submitted documentation. The QEIIMC Parking Department takes all reasonable steps to protect the identifying information it collects from misuse, unauthorised access and disclosure.

    Due to the restrictions that are in place across the QEIIMC site the Priority Parking Policy is the benchmark for all applications to be based on and as such there is no appeal system in place. The QEII Parking Department assess every parking application that has been approved by your Head of Department and assign parking access in accordance to the Priority Groups outlined in the Priority Parking Policy.

    If you have completed your online application form incorrectly, you are welcome to re-submit a new online application. Ensure that you complete all questions truthfully and take special note to read any details or conditions that may appear in the application steps throughout the questionnaire.

    If you are a Primary Carer, you will need to submit a Statutory Declaration form stating that you are the Primary Carer, who you are a Primary Carer for, respective dates of birth and the nature of your care. NB: Status of parking access will remain until the Statutory Declaration has been assessed, approved and a notification has been sent to you with your updated parking allocation. If not approved, access will not be changed.

    Regular audits are conducted by the QEIIMC Parking Department for existing parkers in attempt to apply fairness and equitability. If you know of any colleagues or staff members that you believe to have been issued everyday parking access that appears to be outside of the Policy, you can email the Parking Department and note your observations so they may be taken into consideration. Any information we receive will be treated as strictly confidential. Similarly, no information about the parker you are informing the Parking Department of will be issued to the enquirer.

Without this supporting document, the applicant will remain in Priority Group 3.

    Wilson Parking provides 24 hour monitoring through CCTV in the Multi Deck Car Park and digital intercoms. Wilson Parking attendants are on site in the Multi Deck Car Park 7 days a week, 24 hours a day. Services include monitoring the barrier gates and pay machines.
